Recognizing and Reporting Impairment in the Workplace for Florida Nurses

 Despite the continual increase in rates of substance abuse across the country, nurses and other health professionals rarely report themselves for treatment of drug or alcohol abuse, even when it impairs their ability to practice. Florida, among other states, has mandated continuing educational credits on recognizing impairment in the workplace for nurses to maintain their licensure. As of the 2010 U.S. Census, the state of Florida employed over 213,000 registered nurses (including advanced practice nurses) and licensed practical nurses. With this in mind, Florida’s population is at risk for having over 2,000 impaired nursing professionals in the workforce, yet there is currently a lack of knowledge regarding the identification of, reporting requirements for, and treatment options available to impaired professionals working in the healthcare arena.

As healthcare professionals, nurses are responsible for identifying the signs and symptoms of impaired practice. They are responsible for documenting and reporting impaired practice and for providing treatment opportunities to assist impaired professionals in their recovery as well as their safe return to practice. This course is designed especially for registered nurses, licensed practical/vocational nurses, and advanced practice nurses and will meet the mandated continuing education requirement set forth by the Florida Board of Nursing to provide education related to recognizing impairment in the workforce. Completing this course will provide nurses with the information needed, specific to the state of Florida, to promote and provide safe practice, facilitate interventions, and protect the public if faced with impairment of a colleague or self.

Dr. Leahy is certified as a child/adolescent and adult family psychiatric advanced practice nurse, specializing in individual, couples, and family therapy as well as  psychopharmacology and pharmacogenetics. She also maintains specialization as a master clinician in psychopharmacology through the Neuroscience Education Institute. Barbara D. Fulcher, RN, CNS, PMH, NP-BC, has over 40 years of professional nursing experience. Her early career was spent in medical and cardiac surgical intensive care unit settings. She continued her education by obtaining a second BS in psychology, a master’s in psychiatric nursing, and certification as a psychiatric nurse practitioner. Her psychiatric experience has included working at the Harvard-affiliated McLean Hospital in Belmont, MA, on an affective disorders unit and as a clinical supervisor of the residential treatment center. Currently, she is the program director and an assistant professor at Florida Gulf Coast University’s Master of Science in Nursing Nurse Anesthesia Program in Fort Myers, Florida. Dr. Spiegel earned a diploma in nursing from the St. Francis General Hospital School of Professional Nursing in 1980; a diploma in nurse anesthesia from the Mary Hitchcock Memorial Hospital School of Nurse Anesthesia at Dartmouth-Hitchcock Medical Center in 1986; a juris doctor degree, magna cum laude, from the Florida International University College of Law in 2004; and a doctor of nurse anesthesia practice degree from Virginia Commonwealth University in 2013. According to the CDC, more than 1 in 4 women and more than 1 in 10 men have experienced sexual violence, physical violence, or stalking by an intimate partner that resulted in significant health consequences. Victims of IPV are significantly more likely to experience adverse health outcomes compared with the general population, and suffer from severe mental health outcomes like depression, posttraumatic stress disorder, substance abuse and suicidality. Victims of trauma are also more likely to seek health services compared with those with no history of victimization. As such, healthcare visits provide an opportune time to identify patients with a history of IPV and to intervene. Identifying experiences of IPV among patients can also help healthcare providers tailor care to better meet their unique needs.

The purpose of this course is to provide Florida nurses and other healthcare professionals with information needed to appropriately care for patients in the clinical setting who may be affected by IPV. Skin cancer is a major public health problem. She is an Institute of Medicine, American Board of Emergency Medicine fellow in health sciences policy and cofounder of HEAL (Health, Education, Advocacy, Linkages) Trafficking, an international network of pro­fessionals combating human trafficking from a public health perspective. Through her work, she seeks to advance research and policy on the health needs of human trafficking victims globally and locally.

Demetrius James Porche, DNS, PhD, APRN, is professor and dean of Louisiana State University Health Sciences Center in New Orleans, Louisiana School of Nursing. He also holds an appointment in the School of Public Health at Louisiana State University Health Sciences Center. Dr. Porche is certified as a clinical specialist in community health nursing and family nurse practitioner. He is currently the chief editor of American Journal of Men’s Health and serves on the editorial board of the Journal for Nurse Practitioners. Dr. Porche is a gubernatorial appointed member of the Louisiana State Board of Nursing. He was inducted as a fellow in the American Academy of Nurse Practitioners and a fellow in the American Academy of Nursing.
